We want to thank you for trusting us with your care. When you schedule your appointment we ask that you provide us with a valid email address and mobile phone number, if possible, so that we may communicate with you about your appointment. This will allow you to receive appointment reminders and complete the check-in process. About a week prior to your appointment, you will receive a link via email and/or text to check in for your appointment. This will allow you to complete your medical history and even pay any copays prior to your appointment. Please take the time to complete the check in before you arrive for your appointment as it will save you time once you get here and allow the staff to get to know you prior to your appointment. If you are unable to complete the check in prior to arrival, please arrive at least 15 minutes prior to your appointment so that you can complete it before your appointment time.

You will receive an email when you arrive at your appointment with information about signing up for our patient portal. Our patient portal is a great tool that allows you to stay fully informed about your visits. With the patient portal you are able to receive copies of your lab results, communicate with our staff and providers, pay your bill online, and more. If you need help signing up, please don't hesitate to check with our front desk staff.
